PERSONAL
ASSISTANCE​

Some individuals we serve require a few hours of assistance each week in their personal homes. We provide one-on-one staff to assist with shopping, daily living skills, community outings, cooking or whatever is needed.
SUPPORTED EMPLOYMENT

Job Training:

We assist individuals in finding and maintaining employment, we provide job training and long term support

We provide support until the employee, job coach and employer feel the employee can work independently

Our job coaches receive intensive training classes that specialize in community support on the job, standard requirements to learn correct approaches to help communicate, train and advocate for our client's success.

Wait time for service is minimal and referrals are received from the Division of Vocational Rehabilitation of NC.

RESIDENTIAL
SERVICES

 

Residential supports and supported living services assist the individuals we serve in maintaining the highest levels of independence possible. These services enable our clients to achieve or maintain the living arrangements they desire, with the supervision that is required.  These settings are managed, supported, and operated by our staff.

These services are provided in licensed/unlicensed community residential settings that include group homes and AFL (Alternative Family Living) homes. Transportation to and from community activities is provided from our residences. 

DAY
SUPPORTS

 

We offer group and individual activities to support personal independence and promote social, physical and emotional well-being.  Our day activity program provides opportunities for volunteer and community involvement.

 

The Servant's Heart day program is strategically located adjacent to Guilford Battleground Park. Our facility has direct access via private sidewalk to the many benefits of this National Military Park. On any given day, outdoor excursions and use of the 7 mile Greenway are available to the individuals we serve, including our non-ambulatory individuals.

 

Our day program encourages personal growth through ever-evolving use of music, art, media and social activities. This access to so many different activities encourages the individual to discover their potential.

US

MISSION STATEMENT

Servant’s Heart has been established, as part of SHFP Inc., to provide quality, person-centered residential, vocational and community-based services that promote dignity, self-determination and independence for adults with disabilities.

OUR 

DESIRE is to see every person have the same opportunities in life, no matter what their abilities or disabilities may be.  We believe that even though a person may have significant challenges, they deserve the dignity of a life lived to its fullest potential.

 

We aim to see everyone reach their highest level of independence while helping them realize his or her full potential as a citizen and a member of a community.  We minister and provide services to people with intellectual and developmental disabilities, mental illnesses or are in other ways challenged.

 

OUR

COMMITMENT is to meet the total needs of the individual in residential, social, educational, physical and spiritual ways.

We are a community of parents, guardians, consultants, staff, and management who are committed to:

 

  • Fully trained direct service staff.  Annual training in 1st aid, CPR, blood-borne pathogens, medication administration, NCI, Client rights, confidentiality, annual driver license, and criminal background checks.

  • Encourage and support individuals to achieve their full potential, afford the dignity of risk, encourage and support to live, learn, work, play, and worship of their choosing.

  • Participate in all aspects of the planning, implementation, monitoring, and evaluation of services and supports.

  • Have access to coordinated services and supports that is determined by the individual's unique strengths, needs, and choices.

  • Minimal wait time for services (30-60 days)

A CARF 3-Year Accreditation was awarded to Servant's Heart for the following services:

 

  • Day Program

  • Residential Program

  • Supported Employment

  • Job Training

 

What is CARF?

 

CARF accreditation demonstrates quality, accountability, and commitment to the satisfaction of the persons served.

 

CARF International is an independent, nonprofit accreditor of health and human services providers in the areas of Aging Services, Behavioral Health, Child and Youth Services, Durable Medical Equipment, Employment and Community Services, Medical Rehabilitation, Opioid Treatment Programs, and Vision Rehabilitation Services.
